چکیده

Oral anticoagulant therapy (OAT) calls for continuous control by prothrombin time (PT) test, as the therapeutic range in INR units is very narrow. Warfarin (or coumarin) inhibits coagulation factor synthesis in the liver, but at the same time inactive coagulation factors are formed. The aim here was to measure "active coagulation factors" and inhibition in calibrator kits and patient plasmas by a new method for Quick and Owren PT. Four calibration kits and 200 plasma samples obtained from OAT patients were assessed using Quick and Owren PT for INRTot (active coagulation factors + inhibition) and INRAcf (only active coagulation factors). Conspicuous variation in inhibition was noted between the four calibration kits. The newgeneration PT method develops anticoagulation therapy based on active coagulation factors in vivo and improves INR result harmonization for Quick and Owren PT reagents. This new approach improves Quick PT reliability.

A sensitivity comparison of the Quick and Owren prothrombin time methods in oral anticoagulant therapy

Prothrombin time (PT) is the leading test for monitoring oral anticoagulation therapy (OAT). According to the World Health Organization recommendation, International Normalized Ratio (INR) results obtained from the same patient samples with the major PT methods (Quick and Owren) should be the same when the therapeutic range is the same. In our study blood samples were obtained from 207 OAT pati...

INR calibration of Owren-type prothrombin time based on the relationship between PT% and INR utilizing normal plasma samples.

Prothrombin time (PT) is clinically important and is used to monitor oral anticoagulant therapy. To obtain PT results in international normalized ratio (INR), the current standardization procedure is complex and involves reference reagents. The PT of diluted plasma samples can be determined with a combined thromboplastin (the Owren-type procedure), but not necessarily with a plain thromboplasti...

The progress of prothrombin time measurement

Warfarin is the most widely used medicine for oral anticoagulant therapy (OAT). It inhibits the synthesis of coagulation factors II, VII, IX, and X in the liver and results in the production of inactive or partially active versions of these factors. Inactive coagulation factors interfere with prothrombin time measurement (Quick and Owren PT) measuring the sum of coagulation activity and inhibit...

Effects of the oral, direct thrombin inhibitor dabigatran on five common coagulation assays.

Dabigatran is an oral, reversible thrombin inhibitor that has shown promising results in large clinical trials. Laboratory monitoring is not needed but the effects on common coagulation assays are incompletely known. Dabigatran was added to plasma from healthy subjects in the concentration range 0-1,000 μg/l and analysed using several reagents for activated thromboplastin time (APTT), prothromb...

Has the time arrived to replace the quick prothrombin time test for monitoring oral anticoagulant therapy?

The Quick (1 ) and Owren (2 ) prothrombin time (PT) tests remain the basis for monitoring anticoagulant therapy worldwide. The use of these tests is remarkable because the Quick PT preceded the discovery of the anticoagulant effect of dicoumarol in spoiled sweet clover and both tests were in use long before there was any understanding of the mechanism of the action of oral anticoagulants or whi...
